What personal data we collect and why we collect it

Membership Applications

The Club will collect the following from our members:

Name, postal address, phone number, email address.

How will the data be collected?

The data will be collected via an annual member’s paper application form or electronically via the website.

How is the data to be stored?

The paper membership forms will be stored in a binder held by the Membership Secretary of the Club, in addition an electronic address book will be stored on the Membership Secretary’s Computer System which is security, and password protected. Only the current subscription paid member’s form are to be held.

The same information will be held on the Member’s section of the Website which will be accessed by the Webmaster and the Membership Secretary. This is also security and password protected.

How will the data be used?

The data will be used to notify members of Club’s business, only with member’s express permission, as indicated by individual member’s preferences, and agreed by the individual member.
All group mailings to members of the Club will be sent via “Blind Carbon Copy” (bcc) to ensure email confidentiality.

How will the data be destroyed?

When the Membership Secretary is notified of a member resigning from the Club, contact details will be removed upon request.

Contact forms

When visitors use a Contact Form on the site we collect the data shown in the contact form, and also the visitor’s IP address and browser user agent string to help spam detection.


If you register on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you visit again. These cookies will last for one year.

If you have an account and you log in to this site, we will set a temporary cookie to determine if your browser accepts cookies. This cookie contains no personal data and is discarded when you close your browser.

For users that register on our website (if any), we store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.

What rights you have over your data

If you have an account on this site you can erase any personal data we hold about you in your personal profile. This does not include any data we are obliged to keep for administrative, legal, or security purposes.

The data the software collects

In order to distinguish individual users, IP addresses are collected and stored with the responses.

Each individual form may have fields for a variety of other personal information, such as name and email. This data is needed to identify and possibly communicate with the user. There may be other fields asking for personal information and this data may be for different purposes for each quiz, survey, or form. If any data is to be used for purposes other than grading or survey purposes, this will be disclosed on the form itself.

How long we retain your data

The responses and data attached to the responses are stored indefinitely until an administrator of this site deletes the responses.

Quiz And Survey Master does not send any of your data to anywhere outside of this site by default.
